Illinois is one of the states where CMS’s own fact sheet and our count of 2027 plan filings agree: 10 standalone drug plans, offered across all 102 counties. Medicare Plan Finder counts can still differ slightly. The bigger 2027 change is on the Medicare Advantage side, where Illinois loses 21 plans.

Two features make the Illinois list a little different from many other states: HealthSpring sells two plans here instead of one, and there is no state drug program behind Part D since Illinois Cares Rx ended. Federal 2027 rules are on the Medicare Part D plans 2027 hub.

Key Takeaways

  • CMS lists 10 standalone Part D plans for Illinois in 2027 and a lowest premium of $5.30 a month.
  • HealthSpring files both Assurance Rx and Extra Rx in Illinois; Extra Rx appears in only a handful of Part D regions in our count.
  • Illinois Medicare Advantage plans fall from 157 to 136, while the average MA premium drops to $7.11.
  • Illinois Cares Rx, the old state drug subsidy, ended June 30, 2012, so Extra Help is the main source of cost relief.
  • The Senior Health Insurance Program at the Illinois Department on Aging offers free counseling.

CMS 2027 Medicare Advantage and Part D landscape state fact sheet. Our count of CMS 2027 filings shows 134 individual MA plans, a small difference from counting rules.

The $5.30 floor matches the lowest premium CMS reports in most states for 2027. A low premium is only one part of the bill. The deductible can run up to $700, and drugs on a high tier may cost far more on one plan than another.

Two plans from the same sponsor can differ in premium, deductible, preferred pharmacies and which drugs sit on which tier. Check each plan’s 2027 drug list rather than assuming one is a richer version of the other. A drug covered by one plan may be missing from another.

Spreading 2027 Drug Costs Across the Year

The Medicare Prescription Payment Plan lets you pay your Part D pharmacy costs in monthly bills instead of all at once at the counter. It continues in 2027, and 2026 participants who stay in the same plan are automatically re-enrolled. The monthly bill is capped based on how much of the $2,400 out-of-pocket limit is left and how many months remain.

$200

maximum monthly bill if you start in January with $2,400 in costs ahead (our math)

$400

maximum monthly bill if you join in July with no costs yet (our math)

$0

pharmacy cost for covered drugs after you reach $2,400 (CMS)

Timing tip: the payment plan spreads costs over fewer months the later you sign up, so opting in before January spreads them across all 12. It does not lower the total you owe. Details are on our 2027 payment plan page.

What Happened to Illinois Cares Rx

Program ended: the Illinois Department of Healthcare and Family Services eliminated Illinois Cares Rx effective June 30, 2012, and members lost the state subsidy starting July 1, 2012. Older guides that still mention it are out of date.

Without a state wraparound, Illinoisans with limited income should look at federal Extra Help, which lowers premiums, deductibles and copays in any Part D plan. People with both Medicare and full Medicaid get Extra Help automatically.

Illinois Leans Toward PPOs in Medicare Advantage

Our count of CMS 2027 filings Illinois
Individual MA plans 134
Plan type mix PPO 48%, HMO 32%, HMO-POS 19%, PFFS 1%
Special needs plans 30, mostly chronic-condition SNPs
Most plans by county Cook 65, Will 61, DuPage 59, Kane 56
Fewest plans by county Saline 14, Randolph 13, Williamson 7
Largest MA sponsors Humana 30, UnitedHealthcare 24, Devoted Health 24, Aetna 21

Standalone Part D plans do not vary by county; all 10 are offered statewide.

PPOs generally cover some care outside the network, usually at a higher cost share, so Illinois MA members have more provider flexibility than in HMO-heavy states. In Williamson County, with only 7 MA plans in our count, a standalone drug plan with Original Medicare may be the more flexible route.

The Bottom Line

The Bottom Line

Illinois gets 10 standalone Part D plans for 2027, a figure CMS and our count of plan filings agree on, with premiums starting at $5.30 a month. HealthSpring’s two-plan lineup is the local quirk, and the end of Illinois Cares Rx means Extra Help is the main safety net. The state’s SHIP can compare plans for your drugs at no cost.
