Standalone Part D plans are sold by region, not by county, so everyone in a state usually sees the same list of drug plans. That makes a state-by-state count useful: it tells you how many standalone choices you will have before you even enter a ZIP code. Medicare Advantage plans with drug coverage are different and vary county by county.
We counted every individual (non-employer) standalone prescription drug plan in the CMS 2027 plan benefit files and matched each one to the states it serves. The table below is the result. For the 2027 rules every plan follows, see our Medicare Part D plans 2027 guide and the Part D premium 2027 breakdown.
Key Takeaways
CMS divides the country into 34 Part D regions. Most are a single state; some combine several smaller states, such as Iowa, Minnesota, Montana, Nebraska, North Dakota, South Dakota and Wyoming, which share one region. A standalone plan approved for a region is available to everyone living there, at the same premium.
Standalone Part D plan (PDP)
Sold by region, usually statewide
Same premium across the region
Pairs with Original Medicare and a Medigap plan
Medicare Advantage with drugs (MA-PD)
Sold county by county
Includes medical and drug coverage

Our count of individual standalone Part D plans in the CMS 2027 plan benefit files, by the region each plan serves. Employer plans excluded. Medicare Plan Finder counts can differ slightly.
Plan-to-plan differences come down to the Part D formulary, the 2027 deductible and premiums.
Humana
102 filings; Basic Rx, Value Rx and Premier Rx in all 34 regions
UnitedHealthcare
73 filings; Medicare Rx Preferred and Medicare Rx Saver
Wellcare
68 filings; Classic and Value Script
HealthSpring
40 filings; Assurance Rx in every region plus Extra Rx in a few
Aetna’s standalone business runs through SilverScript Choice, filed in 20 regions for 2027. HealthSpring is the former Cigna Medicare business, now owned by Health Care Service Corporation. Ten regional Blue Cross Blue Shield companies add their own standalone plans in their home states, including Florida Blue, Blue Shield of California and Horizon in New Jersey.
Plan names are not plan costs: two plans with the same name can have different premiums and deductibles in different regions. Always compare the plan in your own state.

| Group | States | Why |
|---|---|---|
| Most (11-12 plans) | Kansas, Alabama, Arkansas, Florida, Michigan, New Jersey, North Carolina, Tennessee | A local Blue plan adds two or three standalone plans on top of the national companies |
| Middle (9-10 plans) | California, Texas, Illinois, New York, Ohio, Georgia, Virginia and others | All five national sponsors, or four plus a Blue plan |
| Fewest (7-8 plans) | Maine and New Hampshire (7), plus 22 states and D.C. with 8 | Aetna SilverScript is not filed in the region and there is no regional Blue plan |

Fewer standalone plans does not mean fewer drug coverage options overall. Most counties also have many Medicare Advantage plans with drug coverage. CMS reports 5,532 Medicare Advantage plans for 2027 nationally, and 97% of people with Medicare can choose from 10 or more.

Start with your exact drug list. Write down each drug, dose and how often you take it.
Enter your ZIP code. That pulls up the standalone plans for your region and the Medicare Advantage plans for your county.
Check each plan’s formulary and tier. A covered drug on a high tier can cost more than a low-premium plan saves.
Compare yearly cost, not monthly premium. Add 12 months of premiums to the estimated drug costs.
Check your pharmacy. Preferred pharmacies in a plan’s network often have lower copays.
Note: not every plan is available in every area, and no single comparison tool lists every plan from every company. Medicare.gov’s Plan Finder shows all Medicare-approved plans for your ZIP code.
Our Part D open enrollment 2027 page covers the deadlines.
Yes, if you move outside your plan’s region. A standalone plan only serves the region it is approved for, so moving from, say, Florida to Georgia means choosing a new plan. Moving gives you a Special Enrollment Period to switch, which generally starts the month before you move and lasts two months after.
Same company, new plan: even if your company offers plans in the new state, the plan, premium and sometimes the formulary will be different. Compare again after any move.
The Bottom Line
For 2027 every state has between 7 and 12 standalone Part D plans in our count, from four to six companies, with Kansas at the top and Maine and New Hampshire at the bottom. Compare the plans in your own state by total yearly cost before December 7, 2026.

Our count of CMS 2027 plan filings shows 7 to 12 standalone Part D plans per state. Your county may also have many Medicare Advantage plans with drug coverage.
Kansas has the most in our count, with 12, because Blue Cross and Blue Shield of Kansas adds three plans to the national companies.
No. Standalone plans are approved by region, so the companies, premiums and deductibles differ by state. The federal rules, such as the $2,400 cap, are the same everywhere.
Humana, UnitedHealthcare, Wellcare and HealthSpring each file standalone plans in every state in our count.
No. Aetna filed SilverScript Choice in 20 regions for 2027, so some states do not have an Aetna standalone plan.
CMS combines some smaller states into one Part D region, such as Idaho and Utah or Oregon and Washington, so they share the same standalone plans.
If you leave your plan’s region you need a new plan. A move gives you a Special Enrollment Period to choose one.
CMS says the lowest standalone Part D premium is $5.30 a month in most states for 2027. The cheapest plan may not cover your drugs, so compare total cost.
