Extra Help Medicare Part D 2027: Who Qualifies and What It Saves

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 30, 2026

Extra Help is the federal program that pays most Medicare Part D costs for people with limited income and savings, and it continues in 2027. If you qualify, you pay no deductible, small fixed copays, and often a $0 premium if you pick a plan whose premium is at or below your region’s Extra Help limit.

Extra Help, also called the Low-Income Subsidy or LIS, is run by Medicare and Social Security. It is not a separate plan. It is a discount that sits on top of whatever Part D plan you are in, so it lowers the bills you get from that plan.

The 2027 Part D rules raise the standard deductible to $700 and the out-of-pocket cap to $2,400, which makes Extra Help worth more than ever for people who qualify. This page shows the current limits, does the copay math, and explains how Extra Help works with your plan choice. The other 2027 rule changes are summarized in our Medicare Part D plans 2027 guide.

Key Takeaways

  • Extra Help covers the Part D deductible, so a member never pays the $700 standard deductible.
  • It pays all or most of the premium for plans at or below a regional benchmark amount.
  • Copays are capped at a few dollars per prescription, set by CMS each year.
  • People with full Medicaid, a Medicare Savings Program or SSI get Extra Help automatically.
  • Extra Help members may change drug plans once a month, effective the first of the next month.

What Extra Help Pays For in 2027

Extra Help attacks the three parts of a Part D bill at once: the monthly premium, the yearly deductible, and the price you pay at the pharmacy. Without it, a person on several brand-name drugs could pay a $700 deductible and then 25% of the drug cost until reaching $2,400. With it, those costs shrink to small copays.

Extra Help does not change which drugs a plan covers. You still need to check that your medicines are on the plan’s drug list, which our Medicare Part D formulary 2027 page explains.

Income and Resource Limits

Medicare publishes new limits each year. The figures below are the 2026 limits on Medicare.gov, which are the ones Social Security uses for applications right now. Medicare says the limits may go up each year, so the numbers for 2027 applications may be a little higher.

Household Yearly income limit Resource limit
Single person $23,940 $18,090
Married couple living together $32,460 $36,100

Tip: Apply even if you are a little over a limit. Social Security subtracts some income and counts it its own way, and only the official review can say whether you qualify.

Our Copay Math: Extra Help vs. a Standard 2027 Plan

Extra Help copays are fixed dollar amounts, one for generic drugs and one for brand-name drugs. CMS sets new amounts each year in a memo that usually comes out in the fall. The 2027 amounts were not yet posted when we checked, so the math below uses the 2026 CMS amounts. The 2027 copays may be a few cents higher.

Who you are Generic copay (2026) Brand copay (2026) 3 generics + 2 brands a month Same, for a year
Full Medicaid, in a nursing home or using home and community services $0 $0 $0.00 $0.00
Full Medicaid, income at or below 100% of poverty $1.60 $4.90 $14.60 $175.20
All other full Extra Help members $5.10 $12.65 $40.60 $487.20

Copays from the CMS CY 2026 LIS memo. Monthly cost is our math: 3 x generic + 2 x brand. Copays apply per prescription fill.

Compare that with a standard 2027 benefit. A person with the same five prescriptions who uses brand-name drugs could pay the full $700 deductible early in the year and then keep paying 25% of each drug’s cost until reaching the $2,400 cap. On the full Extra Help row, the same person’s yearly copays come to $487.20, which is about one-fifth of the cap.

How the Premium Help Works

For premiums, Extra Help pays up to a set amount in each of the 34 Part D regions. That amount is called the low-income benchmark. If a plan’s basic premium is at or below the benchmark, a full Extra Help member usually pays $0. If you choose a plan that costs more, you pay the difference.

Benchmark plan

Premium at or below the regional limit: usually $0 to a full Extra Help member

Pricier plan

You may pay the amount above the benchmark

Enhanced plan

Extra Help covers the basic part; you may pay the extra-benefit part

Which plans are benchmark plans changes every year and depends on the region. A plan that cost you $0 in 2026 may not be $0 in 2027. Medicare usually sends a letter in the fall if your plan’s premium will no longer be fully covered. The premium side of 2027 is covered in more depth on our Medicare Part D premium 2027 page.

Who Gets Extra Help Automatically

Many people never fill out an application. Medicare automatically gives Extra Help to people who already get certain other help, and it mails them a colored letter to confirm it.

You qualify automatically if you have

✔

Full Medicaid coverage

✔

A Medicare Savings Program that pays your Part B premium

✔

Supplemental Security Income (SSI) payments

If you qualify automatically but do not pick a drug plan, Medicare may enroll you in a benchmark plan in your area so you are not left without coverage. You can switch away from that plan if it does not cover your drugs well. Medicaid questions are covered on our if I have Medicaid, do I need Medicare Part D page.

Extra Help and Your 2027 Plan Choices

Extra Help members have more freedom to change drug plans than most people. Since 2025, people with Extra Help or Medicaid can switch to a different standalone drug plan, or move from Medicare Advantage to Original Medicare with a standalone drug plan, once per calendar month. The change starts on the first of the next month. This monthly option cannot be used to join a Medicare Advantage plan. The full set of change windows is on our when can you change Medicare Part D plans 2027 page.

Our count of CMS 2027 plan filings for individual standalone PDPs. Medicare Plan Finder counts can differ slightly. Benchmark status varies by plan and region.

Extra Help also stops the late enrollment penalty. Medicare does not bill the penalty while you have Extra Help, and months on Extra Help are not counted. See our Medicare Part D late enrollment penalty 2027 page for the math.

The Bottom Line

The Bottom Line

Extra Help is the largest discount in Medicare Part D, and in 2027 it shields members from the $700 deductible and most of the cost before the $2,400 cap. If your income and savings are near the limits, apply with Social Security, then check that your 2027 plan still covers your drugs and is still a $0-premium benchmark plan in your region.

Frequently Asked Questions

➤ Is Extra Help still available for Medicare Part D in 2027?

Yes. Extra Help, also called the Low-Income Subsidy, continues in 2027. It lowers premiums, removes the deductible and caps copays for people who qualify.

➤ What are the Extra Help income limits for 2027?

Medicare updates the limits each year. The current 2026 limits are $23,940 a year for one person and $32,460 for a married couple, and the 2027 limits may be slightly higher.

➤ Do I pay the $700 Part D deductible if I have Extra Help?

No. People with Extra Help do not pay a Part D deductible. They pay only the small Extra Help copays from the first fill.

➤ Will my Part D premium be $0 with Extra Help?

It may be. Full Extra Help usually covers the whole basic premium of a plan at or below your region’s benchmark amount. If you pick a costlier or enhanced plan, you may pay part of the premium.

➤ Do I have to reapply for Extra Help every year?

Not usually. Social Security may review your case and send a form to fill out. People who qualify automatically through Medicaid or SSI are rechecked by Medicare each year.

➤ Can I change Part D plans in the middle of the year with Extra Help?

Yes. People with Extra Help can switch standalone drug plans once per calendar month, with the new plan starting the first of the next month. This option cannot be used to join a Medicare Advantage plan.

➤ Does Extra Help remove my late enrollment penalty?

Medicare does not charge the late enrollment penalty to people with Extra Help, and months on Extra Help do not count toward it. If you lose Extra Help later, a past penalty may be billed again.

➤ Does Extra Help pay for drugs my plan does not cover?

No. Extra Help lowers costs only for drugs on your plan’s drug list. If a drug is not covered, you can ask the plan for an exception or look for a plan that covers it.
