What Is Medicare Part D in 2027? Coverage, Costs and Who Offers It

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 30, 2026

Medicare Part D is the part of Medicare that helps pay for prescription drugs, and in 2027 it is sold only through private insurance companies approved by Medicare. You can get it as a standalone drug plan next to Original Medicare or built into a Medicare Advantage plan, and in 2027 your yearly out-of-pocket cost for covered drugs is capped at $2,400.

Original Medicare, meaning Part A for hospital stays and Part B for doctor visits, does not pay for most pills you pick up at a pharmacy. Part D fills that hole. Medicare sets the ground rules and pays a large share of the cost, but you buy the coverage from a private company and pay that company a monthly premium.

This page explains how Part D works for 2027 with real numbers: who sells it, what a year of coverage may cost, and what protections come with it. For the full list of 2027 rule changes, see our Medicare Part D plans 2027 guide, and for premiums by plan type, see Medicare Part D premium 2027.

Key Takeaways

  • Part D is optional, but skipping it without other good drug coverage can lead to a lifelong penalty.
  • In our count of CMS 2027 filings, 15 companies filed 323 standalone plans, with 7 to 12 per state.
  • For 2027 you pay up to a $700 deductible, then about 25% of drug costs, then $0 after $2,400.
  • Covered insulin is capped at $35 a month and recommended adult vaccines cost $0.
  • Each plan has its own drug list, so not all plans cover every drug you take.

Medicare Part D Explained in Plain Words

Think of Part D as drug insurance with federal guardrails. Each plan keeps a list of covered drugs, called a formulary, sorts them into price tiers and works with a network of pharmacies. Medicare requires each plan to cover drugs in key classes and to meet a minimum value, but plans can choose which specific drugs and which prices within those rules.

What Part D generally covers

✔

Brand-name and generic prescription drugs on the plan’s formulary

✔

Insulin you inject with a pen or syringe (insulin used in a traditional pump falls under Part B)

✔

Adult vaccines recommended by the CDC’s advisory committee, such as shingles and Tdap

✔

Drugs from retail pharmacies in the plan’s network and, in many plans, mail order

Part D does not cover drugs given in a doctor’s office or hospital outpatient setting, like many infusions. Those generally fall under Part B instead.

Two Ways to Get Part D Coverage in 2027

Standalone drug plan (PDP)

✔

Added next to Original Medicare

✔

Often paired with a Medigap policy

✔

Separate monthly premium, about $36 on average for 2027

✔

Works with any doctor who takes Medicare

Medicare Advantage with drugs (MA-PD)

✔

Drug coverage built into one plan

✔

Drug part of the premium averages $7.00 for 2027

✔

Uses the plan’s network of doctors and hospitals

✔

Benefits and costs vary by plan and county

KFF reports that 56% of all Part D enrollees were in Medicare Advantage drug plans in 2026. Which path fits you depends on how you get your medical coverage, not only on your drugs.

Who Sells Standalone Part D Plans for 2027

We counted the 2027 standalone plan filings in CMS’s plan files. A filing is one plan in one of Medicare’s 34 PDP regions, so a company that offers the same plan in all regions shows up 34 times. Medicare Plan Finder counts can differ slightly from ours.

Company 2027 PDP filings 2027 plan names
Humana 102 Basic Rx, Premier Rx, Value Rx
UnitedHealthcare 73 Medicare Rx Preferred from UHC, Medicare Rx Saver from UHC
Wellcare 68 Wellcare Classic, Wellcare Value Script
HealthSpring (Health Care Service Corporation) 40 HealthSpring Assurance Rx, HealthSpring Extra Rx
Aetna (SilverScript) 20 SilverScript Choice
10 other companies, mostly regional Blue plans 20 Varies by state

Our count of CMS 2027 plan filings (323 total, 15 companies). Plans offered in your ZIP code may be fewer.

Five national companies account for more than 90% of standalone filings. Regional Blue Cross Blue Shield companies make up most of the rest, and they usually sell only in their home states.

What Medicare Part D Costs in 2027

~$36/mo

average standalone plan premium (CMS)

$700

highest deductible a plan may set (CMS)

25%

standard share after the deductible (CMS)

$2,400

most you pay for covered drugs in 2027 (CMS)

Two kinds of cost add up. The premium is the same every month whether you fill a prescription or not. Cost sharing is what you pay when you pick up a drug: first the deductible, then a copay or coinsurance until the $2,400 cap, then nothing. CMS says the lowest standalone premium is $5.30 in most states, and 88% of people without Extra Help can find a basic plan at $10.30 or less, but low-premium plans may have higher drug costs.

People with higher incomes also pay an IRMAA surcharge on top of the premium. The coverage stages are broken down on our Part D coverage phases 2027 page.

Our Math: A Year of Part D on Three Drug Budgets

To show what Part D may cost in a full year, we ran three drug budgets through the standard 2027 design with the average standalone premium of about $36 a month. Real plans use tier copays and may charge less for generics, so treat these as ranges, not quotes.

Monthly drug cost at plan price Drug costs you pay Premiums (12 x $36) Total for 2027
$20 (one or two generics) $240 $432 $672
$300 (a brand drug plus generics) $1,425 $432 $1,857
$1,500 (a specialty drug) $2,400 (cap reached in May) $432 $2,832

Our calculation from CMS 2027 standard-benefit parameters ($700 deductible on every drug, 25% coinsurance, $2,400 cap). Many plans skip the deductible on generic tiers, which would lower the first row.

What this shows: for light drug users the premium is most of the bill, so a low-premium plan that covers your generics may cost least. For heavy users the cap limits drug costs, so drug coverage and pharmacy access matter more than the premium.

Do You Need Part D If You Take Few Drugs?

Many people who take one or two generics wonder whether Part D is worth it. The answer usually turns on the late enrollment penalty and on what could happen later. If you go 63 days or more without Part D or other creditable drug coverage after you first become eligible, a penalty is added to your premium for as long as you have Part D.

1

Check other coverage. Coverage from an employer, union, TRICARE or the VA may count as creditable; your plan sends a notice each year saying whether it does.

2

Price the cheapest plan that covers your drugs. A low-premium plan can cost less per year than many people expect, depending on the area.

3

Think about a new diagnosis. Costly drugs often start without warning, and you can usually join only during set enrollment periods.

Penalty math for 2027: the penalty is 1% of the $41.33 base premium for each full month you went without coverage. Twenty months uncovered adds about $8.30 a month, for life.

The Bottom Line

The Bottom Line

Medicare Part D is prescription drug coverage sold by private companies under Medicare rules, either as a standalone plan or inside a Medicare Advantage plan. For 2027, 15 companies filed standalone plans in our count, and the cost structure is a deductible up to $700, about 25% after that, and $0 once you reach $2,400.

Frequently Asked Questions

➤ What is Medicare Part D in simple terms?

It is Medicare’s prescription drug coverage. You buy it from a private insurance company approved by Medicare and pay a monthly premium plus your share of drug costs.

➤ Is Medicare Part D free in 2027?

Usually not. Standalone plans average about $36 a month for 2027, and KFF found no $0-premium standalone plans for people without Extra Help. Some Medicare Advantage plans include drug coverage with no added premium, depending on the plan and area.

➤ Who offers Medicare Part D plans in 2027?

In our count of CMS 2027 filings, 15 companies filed standalone plans. The largest by filings are Humana, UnitedHealthcare, Wellcare, HealthSpring and Aetna’s SilverScript.

➤ Is Part D required?

No, it is optional. But if you go without it or other creditable drug coverage for 63 days or more after you are first eligible, you may pay a late enrollment penalty for as long as you have Part D.

➤ What is the difference between Part D and Medicare Advantage?

Part D is drug coverage only. Medicare Advantage replaces Original Medicare for medical care, and most Advantage plans include Part D drug coverage in the same plan.

➤ Does Part D cover all prescription drugs?

No. Each plan has its own formulary, and not all plans cover every drug. Check that each of your drugs is on a plan’s 2027 list before you join.

➤ What is the most I can pay for drugs with Part D in 2027?

$2,400 for covered drugs. After that you pay $0 for covered Part D drugs for the rest of 2027. Premiums are extra and do not count toward the cap.

➤ When can I sign up for Part D for 2027?

Most people join when they first get Medicare or during open enrollment, October 15 to December 7, 2026, for coverage starting January 1, 2027. Some life events allow other times.
