Blue Cross Blue Shield Medicare Part D coverage for 2027 is sold by separate local Blue companies, and in our count of CMS 2027 plan filings, 10 of them offer 20 standalone drug plans covering 20 states. In many other states, the local Blue company does not file its own standalone drug plan for 2027, so Blue drug coverage there may come only through a Blue Medicare Advantage plan, depending on the area.

Blue Cross Blue Shield is not one insurance company. It is an association of independent companies, each licensed to use the Blue name in its own state or group of states. That is why there is no single national Blue Part D plan, and why the answer to “Does Blue Cross sell a drug plan?” depends on where you live.
We went through the CMS 2027 plan files and matched each Blue standalone drug plan to the company that filed it and the states it covers. Medicare Plan Finder counts can differ slightly from ours. For the national 2027 rules, see our Medicare Part D plans 2027 hub.
Key Takeaways
Each Blue company decides on its own whether to sell a standalone Part D plan, what to name it and what to charge. A Blue card from one state does not make you eligible for another state’s Blue drug plan. You can join only plans that serve the area where you live.
What each local Blue company sets for 2027
Whether it offers a standalone drug plan at all
The plan names and how many plans it files
The formulary, tiers and copays
The pharmacy network and any preferred pharmacies
The monthly premium in its service area
The table lists every Blue-branded standalone drug plan in our count of CMS 2027 plan filings, grouped by the company that filed it. Availability can still vary by county, so confirm with your ZIP code.

Across the ten companies, a few patterns stand out that no single Blue website shows.
8 of 10
Blue companies filed exactly two standalone plans
3
plans from Blue Cross and Blue Shield of Kansas, the most of any Blue company
7
states share one plan, MedicareBlue Rx Standard
The two-plan pattern usually means one lower-premium plan and one with richer benefits, though names alone do not tell you which costs less for your drugs. Shared plans, like MedicareBlue Rx in the upper Midwest and Blue MedicareRx in New England, are run jointly by several Blue companies, so the same plan name appears in more than one state.

Health Care Service Corporation is the parent of Blue Cross and Blue Shield of Illinois, New Mexico, Oklahoma and Texas. It also bought Cigna’s Medicare business, which now operates as HealthSpring. In the CMS plan files, the standalone drug plans tied to those four Blue companies carry the HealthSpring brand, not the Blue name.
What that means: If you live in one of those four states and want a standalone plan from the Blue family, look for HealthSpring Assurance Rx or HealthSpring Extra Rx where offered, rather than a plan with Blue in its name.
Because the HealthSpring plans are sold nationally, we count them separately from the 20 Blue-branded plans on this page.
Outside the 20 states in the table, our count found no Blue-branded standalone drug plan for 2027. That includes several large states such as New York, Pennsylvania, Ohio, Georgia and Virginia. Some Blue companies in those states focus on Medicare Advantage plans that include drug coverage.

Blue companies set their own benefits, but each Part D plan must meet Medicare’s 2027 standards. Plan-level costs vary, so check the Summary of Benefits for any Blue plan you are weighing.

Yes. Every 2027 Part D plan, including each Blue plan, caps covered drug costs at $2,400. After that, you pay $0 for covered drugs for the rest of the year.
