Yes, Jardiance can be covered by Medicare Part D in 2027 for type 2 diabetes, heart failure and chronic kidney disease, subject to each plan’s formulary. Its negotiated Medicare price of $197 for a 30-day supply continues from 2026, and the drug often shares a prescription list with other negotiated drugs, which changes how fast a person reaches the $2,400 cap.

Jardiance (empagliflozin) is a once-a-day SGLT2 inhibitor made by Boehringer Ingelheim. It started as a blood sugar drug and now carries FDA approvals that reach well beyond diabetes, including heart failure and chronic kidney disease. That wider label is one reason so many Medicare members take it.
The useful 2027 question is less whether Part D pays and more what a full year costs when Jardiance is one of several brand prescriptions. Below are side-by-side cost scenarios worked from CMS 2027 rules, plus the product details that can trip people up. The overall plan-year rules are on our Medicare Part D plans 2027 hub.
Key Takeaways
| FDA-approved use | Who it is for |
|---|---|
| Blood sugar control | Adults and children 10 and older with type 2 diabetes, with diet and exercise |
| Lower risk of cardiovascular death | Adults with type 2 diabetes and known heart disease |
| Heart failure | Adults with heart failure, to lower the risk of cardiovascular death and hospital stays |
| Chronic kidney disease | Adults with kidney disease at risk of getting worse, to slow decline |
Summary of the FDA label. A plan may set prior authorization criteria by use.
Because each of these is a medically accepted use, Part D plans are allowed to cover Jardiance for any of them. A plan may still ask your doctor to confirm the diagnosis.
These scenarios are our own math using the 2027 standard benefit ($700 deductible, then 25% coinsurance, $2,400 cap) and CMS-negotiated 30-day prices: Jardiance $197, Eliquis $231 and Entresto $295. Monthly premiums are not counted.
| Prescriptions | Drug cost per month | January bill | Typical later month | 2027 total |
|---|---|---|---|---|
| Jardiance only | $197 | $197.00 | $49.25 | About $1,116 |
| Jardiance + Eliquis | $428 | $428.00 | $107.00 | About $1,809 |
| Jardiance + Eliquis + Entresto | $723 | $705.75 | $180.75 | $2,400 (cap in November, $0 in December) |
Illustrative only. Plans may use tier copays, a reduced deductible or no deductible on preferred brands, which changes every figure.
Pattern in the numbers: Each added negotiated drug raises the yearly total, but the cap stops the climb. With three heart and kidney drugs, December fills cost nothing under the standard design.
Covered by the $197 negotiated price
Jardiance tablets, 10 mg and 25 mg
Separate products with their own pricing
Synjardy and Synjardy XR (empagliflozin and metformin)
Glyxambi (empagliflozin and linagliptin)
Trijardy XR (empagliflozin, linagliptin and metformin)
CMS treats fixed-dose combinations as distinct drugs. That is why Janumet was chosen on its own for 2027 prices instead of sharing Januvia’s 2026 price. If you take a combination pill, the Jardiance price does not carry over, and taking two separate pills could cost a different amount depending on the plan.

Jardiance and Farxiga belong to the same drug class. In April 2026 the FDA approved the first generic versions of dapagliflozin, the active ingredient in Farxiga. Jardiance has no U.S. generic yet, and patent protection is expected to keep it that way for several more years.
Points to raise with your prescriber
Whether a generic SGLT2 drug fits your condition as well as Jardiance
Whether your plan puts generic dapagliflozin on a lower tier
Kidney function and heart failure type, which can steer the choice
That switching is a medical decision, not only a price decision
We compare the Farxiga side on is Farxiga covered by Medicare Part D in 2027.
Tier placement. Often a preferred brand tier, though each plan decides.
Step therapy. Some plans may ask you to try metformin or a generic option first for diabetes use.
Quantity limit. Usually one tablet a day.
Exceptions. Your doctor can request one if Jardiance is not listed or a step rule does not fit your case.

The Bottom Line
Jardiance can be covered by Part D plans in 2027 for diabetes, heart failure and kidney disease at its $197 negotiated price. Alone it costs about $1,116 a year under the standard benefit in our math, while adding other negotiated heart drugs can carry a member to the $2,400 cap by November. Check the formulary, tier and step rules in each 2027 plan you consider.

No. Jardiance is FDA-approved for heart failure and chronic kidney disease as well, and Part D plans can cover it for those uses. A plan may ask for the diagnosis on the prescription.
The negotiated price of $197 for a 30-day supply that began January 1, 2026, continues into 2027. What changes is the deductible and cap around it.
No. Combination pills are separate products. Their 2027 prices come from each plan’s normal pricing.
Not in our standard-benefit example, which totals about $1,116. Taking it with other brand drugs can bring you to the cap.
Not in the U.S. yet. Generic dapagliflozin, a related SGLT2 drug, was approved in April 2026.
Some plans use step therapy, especially for diabetes use. Your doctor can request an exception if the earlier step is not right for you.
Medicare Advantage plans with drug coverage follow the same Part D rules and negotiated price. Copays and tiers may differ.
If your plan applies the deductible to brand drugs, you pay the full negotiated price until $700 is met. Coinsurance usually starts in the spring.
