Texas has 10 standalone Medicare Part D plans for 2027 in our count, offered by Humana, Wellcare, HealthSpring, UnitedHealthcare and Aetna SilverScript. The same 10 plans are sold in Harris County and in the smallest Panhandle counties, because the whole state is one Part D region.

Texas counties, all served by the same standalone plan list
Texas is a big state with uneven Medicare Advantage choices. A Houston or San Antonio resident can pick from more than 90 MA plans, while a few West Texas counties have 3. Standalone drug plans work differently: they are approved for the whole state, so every Texan with Original Medicare sees the same 10 options.
The counts here come from our analysis of the CMS 2027 plan benefit files. Medicare Plan Finder counts can differ slightly. The national Part D rules for 2027 are explained on the Medicare Part D plans 2027 hub.
Key Takeaways
| County | Medicare Advantage plans for 2027 | Standalone Part D plans |
|---|---|---|
| Harris (Houston) | 93 | 10 |
| Bexar (San Antonio) | 92 | 10 |
| El Paso | 83 | 10 |
| Dallas | 78 | 10 |
| Comal | 77 | 10 |
| Ochiltree, Terrell, Scurry | 3 each | 10 |
Our count of CMS 2027 plan filings. Medicare Advantage plans are approved county by county; standalone Part D plans are approved for the whole Texas region.
In counties with only a few MA plans, the standalone list is often where most of the choice is. Pairing a standalone plan with Original Medicare also keeps access to any doctor who accepts Medicare, which can matter in rural areas with thin networks.

HealthSpring Medicare Advantage plans in Texas for 2027 (our count)
HealthSpring is Cigna’s former Medicare business, now under Health Care Service Corporation. In the CMS files, its Texas standalone plans are filed under the Blue Cross and Blue Shield of Illinois, New Mexico, Oklahoma and Texas organization, but they are sold under the HealthSpring name. Texas is one of the few states where HealthSpring Extra Rx is offered alongside Assurance Rx.
Name check: Texas has no standalone plan sold under a Blue Cross Blue Shield brand for 2027 in our count, unlike Florida or California, where the local Blue company files its own drug plans.

Our count of CMS 2027 plan filings. CMS’s own Texas fact sheet may report a different total because of counting rules.
The high share of dual special needs plans reflects how many Texans have both Medicare and Medicaid. People with both get Extra Help automatically, which lowers drug costs in a standalone plan or an MA plan.

The Bottom Line
Every Texas county shares the same 10 standalone Part D plans for 2027 in our count, while Medicare Advantage choices range from 93 plans in Harris County to 3 in a few rural counties. HealthSpring’s two plans and Aetna’s SilverScript round out a market with no Blue-branded PDP. Texans with HIV or kidney failure should also check the state’s own drug programs.
