Do I Need Medicare Part D in 2027? A Decision Guide With Penalty Math

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated November 3, 2026

Medicare Part D is optional in 2027, but you likely need it unless you already have creditable drug coverage from a job, the VA, TRICARE or a similar source. Going without both can add a permanent late penalty to your premium, even if you take no prescriptions today.

2027 by the Numbers

$5.30

lowest 2027 standalone Part D premium in most states (CMS)

$41.33

2027 base premium used to figure the late penalty (CMS)

63 days

longest gap allowed without creditable drug coverage before a penalty builds (Medicare)

No law forces anyone with Medicare to buy drug coverage. Original Medicare simply does not pay for most pills you pick up at a pharmacy, and Congress built Part D with a late penalty to push people to join early rather than wait until they get sick.

That makes the real question whether something else already covers your prescriptions at least as well as standard Part D. The sections below sort common situations, then run the numbers on skipping coverage. Plan options for next year are on the Medicare Part D plans 2027 hub.

Key Takeaways

  • Creditable coverage from an employer, union, VA, TRICARE or federal employee plan lets you delay Part D without penalty.
  • If you are in a Medicare Advantage plan with drug coverage, you already have Part D.
  • Skipping coverage for two years adds about $9.90 a month to your 2027 premium for as long as you have Part D.
  • Our math shows the penalty after two years costs more each year than the lowest-premium plan in most states.
  • Your employer or plan must tell you each year, before October 15, whether its drug coverage is creditable.

Quick Answer by Situation

Your drug coverage today Need to join Part D for 2027?
Only Original Medicare, nothing for drugs Yes, to avoid a penalty and cover future prescriptions
Medicare Advantage plan that includes drugs No, it already contains Part D
Job-based coverage confirmed as creditable Not yet; join when that coverage ends
VA health care drug benefit Not required, since VA coverage is creditable
TRICARE For Life Not required; TRICARE pharmacy coverage is creditable
Full Medicaid with Medicare Medicare enrolls you in a drug plan automatically
Retiree plan that is not creditable Yes, to avoid the penalty

General rules from Medicare and CMS. Confirm your coverage’s creditable status in writing each year.

Medigap policies sold today do not cover prescriptions, so a Medigap plan alone does not count as drug coverage.

What Creditable Coverage Means

Creditable coverage is drug coverage expected to pay, on average, at least as much as the standard Part D benefit. Employers and unions run an actuarial test each year and send members a notice with the result. The notice is due before October 15, ahead of the enrollment season.

Keep the paperwork: Save every creditable coverage notice. If you join Part D later, the plan may ask for proof of the months you were covered, and the notice is the main evidence.

More on reading those letters is on our creditable coverage notice page.

Our Penalty Math for 2027

Months without creditable coverage Added to 2027 monthly premium Yearly cost of the penalty
12 $5.00 $60.00
24 $9.90 $118.80
36 $14.90 $178.80
60 $20.70 $248.40

Our math: 1% of the $41.33 CMS 2027 base premium per uncovered month, rounded to the nearest $0.10. The base premium changes yearly, so the dollar amount will move.

The cheaper path: The lowest standalone premium in most states is $5.30 a month, or $63.60 for 2027. After 24 uncovered months, the penalty alone ($118.80 a year) costs nearly twice that, and it never goes away.

The full rule set, including how to ask for a reconsideration, is on our 2027 late enrollment penalty page.

When You Take No Prescriptions

Healthy people are the group most tempted to skip Part D. A low-premium plan works like a placeholder: it keeps the penalty clock from running and gives coverage if a new diagnosis shows up mid-year. Without a plan, a person diagnosed in March usually cannot join until the next enrollment season.

Timing If You Decide to Join

1

Turning 65. Enroll during your seven-month Initial Enrollment Period so coverage starts without a gap.

2

Leaving creditable coverage. You get two full months after that coverage ends to join without a penalty.

3

Everyone else. Use the Annual Enrollment Period, October 15 to December 7, 2026, for coverage that starts January 1, 2027.

Our count of CMS 2027 plan filings shows between 7 and 12 standalone plans available in each state, so most people have several to compare. Medicare Plan Finder counts can differ slightly.

The higher base premium means each uncovered month adds a little more to the penalty than it did in 2026. The higher cap still limits what anyone with Part D pays for covered drugs in a bad year.

The Bottom Line

The Bottom Line

You do not have to buy Part D in 2027, but unless another source gives you creditable drug coverage, skipping it is usually the more expensive choice over time. Even a low-premium plan protects against a lifelong penalty and a surprise diagnosis. Check your coverage notice, then look up 2027 plans in your ZIP code before December 7, 2026.

Frequently Asked Questions

➤ Is Medicare Part D mandatory in 2027?

No. Part D is voluntary, but people who go without it or other creditable coverage for 63 days or more may owe a late penalty when they join.

➤ Does a Medigap plan count as drug coverage?

No. Medigap plans sold today do not include prescription coverage, so they do not protect you from the Part D penalty.

➤ I have employer insurance at 65. Do I need Part D?

Not if the employer’s drug coverage is creditable. Check the yearly notice, and join Part D within two months after that coverage ends.

➤ How is the 2027 penalty figured?

Multiply 1% of $41.33 by the number of full months you went without coverage, then round to the nearest 10 cents. That amount is added to your monthly premium.

➤ Can I drop Part D if I stop taking medicine?

You can, but a later return may carry a penalty for the uncovered months. Keeping a low-premium plan avoids that.

➤ Do I need Part D if I use GoodRx or pharmacy discounts?

Discount cards are not insurance and do not count as creditable coverage, so the penalty can still build.

➤ Does TRICARE For Life replace Part D?

Yes. TRICARE pharmacy coverage is creditable, so most TRICARE For Life members do not need to join a Part D plan.

➤ What if I missed the deadline because of bad advice?

You can ask Medicare for a reconsideration of the penalty within 60 days of the letter. Approval depends on your records and circumstances.
