Blue Cross Blue Shield Medicare Part D 2027: Blue Drug Plans by State

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 31, 2026

Blue Cross Blue Shield Medicare Part D coverage for 2027 is sold by separate local Blue companies, and in our count of CMS 2027 plan filings, 10 of them offer 20 standalone drug plans covering 20 states. In many other states, the local Blue company does not file its own standalone drug plan for 2027, so Blue drug coverage there may come only through a Blue Medicare Advantage plan, depending on the area.

2027 cap on covered Part D drug costs, for Blue plans too (CMS)

Blue Cross Blue Shield is not one insurance company. It is an association of independent companies, each licensed to use the Blue name in its own state or group of states. That is why there is no single national Blue Part D plan, and why the answer to “Does Blue Cross sell a drug plan?” depends on where you live.

We went through the CMS 2027 plan files and matched each Blue standalone drug plan to the company that filed it and the states it covers. Medicare Plan Finder counts can differ slightly from ours. For the national 2027 rules, see our Medicare Part D plans 2027 hub.

Key Takeaways

  • In our count of CMS 2027 filings, 10 Blue companies filed 20 standalone drug plans that cover 20 states.
  • Most Blue companies filed a pair of plans, often a basic option and an enhanced one; Kansas filed three.
  • Some Blue plans are shared across states, such as Blue MedicareRx in Connecticut, Massachusetts, Rhode Island and Vermont.
  • In Illinois, New Mexico, Oklahoma and Texas, the Blue company’s parent, Health Care Service Corporation, sells standalone drug plans under the HealthSpring name.
  • Every Blue plan follows the 2027 federal limits: a deductible of $700 or less and a $2,400 cap on covered drug costs.

Why Blue Part D Plans Change From State to State

Each Blue company decides on its own whether to sell a standalone Part D plan, what to name it and what to charge. A Blue card from one state does not make you eligible for another state’s Blue drug plan. You can join only plans that serve the area where you live.

Patterns in the 2027 Blue Filings

Across the ten companies, a few patterns stand out that no single Blue website shows.

8 of 10

Blue companies filed exactly two standalone plans

3

plans from Blue Cross and Blue Shield of Kansas, the most of any Blue company

7

states share one plan, MedicareBlue Rx Standard

The two-plan pattern usually means one lower-premium plan and one with richer benefits, though names alone do not tell you which costs less for your drugs. Shared plans, like MedicareBlue Rx in the upper Midwest and Blue MedicareRx in New England, are run jointly by several Blue companies, so the same plan name appears in more than one state.

The HealthSpring Link in Illinois, New Mexico, Oklahoma and Texas

Health Care Service Corporation is the parent of Blue Cross and Blue Shield of Illinois, New Mexico, Oklahoma and Texas. It also bought Cigna’s Medicare business, which now operates as HealthSpring. In the CMS plan files, the standalone drug plans tied to those four Blue companies carry the HealthSpring brand, not the Blue name.

What that means: If you live in one of those four states and want a standalone plan from the Blue family, look for HealthSpring Assurance Rx or HealthSpring Extra Rx where offered, rather than a plan with Blue in its name.

Because the HealthSpring plans are sold nationally, we count them separately from the 20 Blue-branded plans on this page.

States Without a Blue Standalone Drug Plan for 2027

Outside the 20 states in the table, our count found no Blue-branded standalone drug plan for 2027. That includes several large states such as New York, Pennsylvania, Ohio, Georgia and Virginia. Some Blue companies in those states focus on Medicare Advantage plans that include drug coverage.

Frequently Asked Questions

➤ Does Blue Cross Blue Shield offer a standalone Part D plan in my state for 2027?

It depends on the state. In our count of CMS 2027 filings, Blue-branded standalone drug plans cover 20 states, including Florida, Michigan, North Carolina, New Jersey and California. Check Medicare Plan Finder with your ZIP code.

➤ How many Blue companies sell standalone drug plans for 2027?

Our count found 10 Blue companies filing 20 standalone drug plans for 2027. Most filed two plans, and Blue Cross and Blue Shield of Kansas filed three.

➤ Why does my Blue plan have a different name than my friend’s in another state?

Each Blue company is independent and names its own plans. A plan called Prescription Blue in Michigan and one called BlueMedicare in Florida come from different companies with different benefits.

➤ Is there a Blue standalone drug plan in Texas or Illinois for 2027?

The Blue companies in Texas, Illinois, New Mexico and Oklahoma belong to Health Care Service Corporation, which sells standalone plans under the HealthSpring name. Our count found no Blue-branded standalone plan in those states.

➤ Can I join a Blue drug plan from another state?

No. You can join only plans that serve the area where you live. If you move, you may need to pick a new plan, and moving gives you a special enrollment period.

➤ Which Blue plan covers several New England states?

Blue MedicareRx covers Connecticut, Massachusetts, Rhode Island and Vermont in our count. It is filed jointly by Anthem and the Blue companies of Massachusetts, Rhode Island and Vermont.

➤ Do Blue drug plans have to follow the $2,400 cap in 2027?

Yes. Every 2027 Part D plan, including each Blue plan, caps covered drug costs at $2,400. After that, you pay $0 for covered drugs for the rest of the year.

➤ What can I do if no Blue standalone plan serves my area?

You can choose a standalone drug plan from another company, or look at a Blue Medicare Advantage plan with drug coverage if one serves your county. Medicare Advantage replaces how your medical care is covered, too.
