A Medicare Part D special enrollment period (SEP) in 2027 lets you join, switch or drop a drug plan outside the fall Open Enrollment Period when a qualifying life event happens. Most SEPs last two full months after the event, but the exact window depends on the event.

Most people can change Part D coverage only in the fall. A special enrollment period is the exception: something changed in your life or in your plan, and Medicare opens a short window just for you. Miss the window and you may have to wait until the next fall, and you could face a late penalty.
This page goes event by event, with the window length and a 2027 date example for each. For a quick look at the other 2027 rules, see our Medicare Part D plans 2027 guide, and for what drug plans cost next year, see Medicare Part D premium 2027.
Key Takeaways
Each qualifying event has its own clock. The table lists the events that come up most often for people with standalone drug plans, with the window Medicare.gov describes for each one.
| What happened | How long the window lasts |
|---|---|
| You moved out of your plan’s service area | Starts when you move (or the month before, if you tell the plan early) and runs 2 full months after |
| You moved within the area and have new plan options | Same as above |
| You moved back to the U.S. from abroad | 2 full months after the month you return |
| Your employer or union coverage ended, including COBRA | 2 full months after the month coverage ends |
| You lost creditable drug coverage through no fault of your own | 2 full months after the loss or the notice, whichever is later |
| You moved into, live in, or left a nursing home or rehab facility | The whole stay plus 2 full months after you move out |
| You were released from jail | 2 full months after release, if your Medicare stayed active |
| You lost Medicaid, or were told Extra Help ends next year | 3 full months from the loss or the notice, whichever is later |
| You have Medicaid or Extra Help | One change per calendar month |
| You dropped a PACE plan or a Medicare Cost Plan | 2 full months after the month you leave |

“Two full months after” is easy to misread. The month of the event does not count as one of the two months. We worked out the last day to act for a few common 2027 situations.
| Event in 2027 | Window closes | If you sign up on the last day, coverage starts |
|---|---|---|
| Employer drug coverage ends March 31 | May 31, 2027 | June 1, 2027 |
| You move to a new state on June 15 | August 31, 2027 | September 1, 2027 |
| Retiree coverage ends September 30 | November 30, 2027 | December 1, 2027 |
| You leave a nursing home on July 20 | September 30, 2027 | October 1, 2027 |
Our date math from the Medicare.gov window lengths. New coverage usually starts the first of the month after the plan gets your request.
Don’t cut it close: If your old coverage ends March 31 and you wait until May 31 to join, you will have two months with no drug coverage. Sign up before the old coverage ends so the new plan starts the next day.
Standalone drug plans are sold by region, and many regions are a single state. When you move across a state line, your current plan may not be offered at your new address, and the number of choices can be quite different.

This is the SEP that protects people who worked past 65. If you kept a job-based plan with creditable drug coverage, you can join Part D when it ends without a late penalty, as long as you use the window.
Get the end date in writing Ask HR or the plan for the exact last day of drug coverage.
Get the creditable letter Ask for written proof that the drug coverage was creditable; you may need it later.
Pick a plan before the end date Join so that Part D starts the day after the old plan stops.
Check COBRA dates If you move to COBRA, the Part D window generally runs from the month that coverage ends. Confirm the COBRA drug coverage is creditable.
If you let the window close, the months after can count toward a penalty once you go 63 days without creditable coverage. Our Medicare Part D late enrollment penalty 2027 page shows what that costs.
Some SEPs are triggered by your plan, not by you. They matter most in the first weeks of 2027.
Plan not renewed for 2027

The 5-star detail is easy to miss. CMS’s 2027 Star Ratings fact sheet, released October 8, 2026, lists no standalone drug plan contract at 5 stars and an average standalone plan rating of 3.22. If you hoped to use the 5-star SEP to move to a different standalone drug plan mid-year, that path may not be open in 2027.

People with Medicaid or Extra Help have a standing monthly SEP. They can switch standalone drug plans, or leave Medicare Advantage for Original Medicare with a standalone drug plan, once each calendar month. The change starts the first of the next month. This option cannot be used to join a Medicare Advantage plan.
Assistance-related SEPs
Monthly SEP for people with Medicaid or Extra Help
3-month window after losing Medicaid or being told Extra Help is ending
Once-a-year SEP for members of a State Pharmaceutical Assistance Program (SPAP)
A window after losing SPAP eligibility
The full benefit and limits are on our Extra Help Medicare Part D 2027 page.
There is no separate SEP application. You use it by joining a plan and answering the question on the enrollment form that asks why you are enrolling now.
Pick the event On the form, check the reason that matches your situation, such as a move or loss of employer coverage.
Add the date Write the date of the event, such as the move date or the last day of employer coverage.
Keep proof handy The plan may ask for a lease, a letter from your employer, or a Medicaid notice.
Watch for the approval letter The plan confirms your start date in writing. Your old drug plan ends when the new one begins.
Not sure you qualify? Contact Medicare and describe the event. Medicare can also grant an SEP for exceptional circumstances on a case-by-case basis.
The Bottom Line
A Part D special enrollment period is tied to a specific event and a specific deadline, usually two full months after the event. In 2027, watch the plan non-renewal window that ends in late February, and know that the 5-star SEP may not help with standalone drug plans because CMS rated none at 5 stars.

Common events include moving out of your plan’s area, losing employer or union drug coverage, entering or leaving a nursing home, and gaining or losing Medicaid or Extra Help. Your plan being ended or sanctioned can also qualify.
Most windows last two full months after the month of the event. Losing Medicaid or being told Extra Help is ending gives three full months, and nursing home residents can change during the whole stay.
It may not help with a standalone plan. CMS’s 2027 Star Ratings list no standalone drug plan contract at 5 stars, so the 5-star SEP may only apply to moving into a 5-star Medicare Advantage plan.
You get an SEP that runs from December 8, 2026 through the last day of February 2027. Join a new plan by December 31, 2026 if you want coverage with no break on January 1.
Usually on the first day of the month after the plan gets your request. Some SEPs let you pick a start date, such as the day after employer coverage ends.
You confirm the event on the enrollment form, and the plan may ask for documents. Keep things like a lease, an employer letter or a Medicaid notice in case they ask.
No. An SEP lets you join or switch plans; it does not remove a penalty. If you think a penalty is wrong, ask for a reconsideration within 60 days of the penalty letter.
You will usually have to wait for the next fall Open Enrollment Period, which runs October 15 to December 7, with coverage starting January 1. Going 63 days or more without creditable coverage can also add a late penalty.
