Find a Medicare Part D prescription drug plan for 2027
Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated November 3, 2026
Massachusetts residents can pick from 9 standalone Medicare Part D plans for 2027 in our count of CMS 2027 plan filings, sold by Humana, Blue Cross Blue Shield of Massachusetts (Blue MedicareRx), Wellcare, HealthSpring and UnitedHealthcare. The bigger change is at the state level: Prescription Advantage stopped taking new applications after September 11, 2026.
2027 by the Numbers

$35,910

For more than two decades, Bay State retirees who missed a cheaper plan could lean on Prescription Advantage, the state’s own drug program that pays after Part D. Mass.gov now says the program will not accept new applications after September 11, 2026. People already enrolled keep their benefits, but anyone joining Medicare from here on has to rely on the plan they choose plus federal and MassHealth help.
That puts more weight on the plan decision itself. Our count of CMS 2027 plan filings finds 9 standalone plans (PDPs) for Massachusetts from 5 sponsors, and Medicare Plan Finder counts can differ slightly. National rules for the year, from the $700 deductible to the $2,400 cap, are collected on the Medicare Part D plans 2027 hub.
Key Takeaways
Enrolled on or before Sept. 11, 2026
Mass.gov says participation continues
Prescription Advantage pays after your Part D plan, as a secondary payer
Members get a yearly special window to join or switch drug plans
Income tiers run from S0 to S5 under the April 2026 rate guide
Turning 65 or new to Massachusetts after that date
New applications are no longer accepted
Your Part D plan is your only drug coverage unless you qualify for MassHealth
Extra Help and the Medicare Savings Program become the main cost relief
The yearly special window tied to the state program is not available
Mass.gov has not said the program is ending for current members, and the 2027 rate schedule was not posted when this page was updated. Members with questions can reach the program through MassOptions. Prescription Advantage has always required Part D enrollment, so a current member who drops drug coverage could lose the state help too.
| Sponsor | 2027 plans filed for Massachusetts | Quick note |
|---|---|---|
| Humana | Basic Rx Plan; Value Rx Plan; Premier Rx Plan | Most plans of any sponsor in the region |
| Blue Cross Blue Shield of Massachusetts | Blue MedicareRx Value Plus; Blue MedicareRx Premier | The only Blue standalone plans in our count |
| Wellcare | Wellcare Classic; Wellcare Value Script | Two lower-cost designs |
| HealthSpring | HealthSpring Assurance Rx | Single plan; HealthSpring is now owned by Health Care Service Corporation |
| UnitedHealthcare | Medicare Rx Saver from UHC | Only one UnitedHealthcare drug plan filed here |
Our count of CMS 2027 plan filings for individual standalone PDPs. Medicare Plan Finder counts can differ slightly, and premiums, pharmacies and drug lists vary by plan.
About Blue MedicareRx: Blue Cross Blue Shield of Massachusetts markets two drug-only plans under the Blue MedicareRx name. In 2025, Premier carried a $0 deductible on every tier while Value Plus charged a deductible on most tiers. The 2027 designs may differ, so check the plan documents. More background is on the Blue Cross Blue Shield Part D 2027 page.
What the shared region means
Our count finds the identical 9-plan list in Connecticut, Massachusetts, Rhode Island and Vermont
A Worcester retiree and a Burlington retiree see the same plan names, though networks of preferred pharmacies can differ
Moving from Massachusetts to Connecticut, Rhode Island or Vermont may not force a plan change, but you must update your address with the plan
The Pittsfield and Provincetown lists are the same, since standalone plans are not sold county by county
Snowbirds who spend winters in Florida are a different case. A standalone plan covers you at network pharmacies nationwide for short trips, but a permanent move out of the four-state region usually means picking a new plan in the new region.

Federal Extra Help on its own looks at savings. Massachusetts takes a gentler path: MassHealth removed the asset limit for its Medicare Savings Program on March 1, 2024, and anyone approved for the MSP qualifies for Extra Help automatically. A retiree with modest income and a healthy bank account may get drug help this way that the federal test alone would deny.
| 2026 figure | Single | Married couple |
|---|---|---|
| Massachusetts MSP income limit (yearly) | $35,910 | $48,690 |
| Massachusetts MSP asset limit | None | None |
| Extra Help income limit if applying directly | $23,940 | $32,460 |
| Extra Help resource limit if applying directly | $18,090 | $36,100 |
2026 figures from the Mass.gov Prescription Advantage rate guide (effective April 1, 2026) and Massachusetts Law Reform Institute; 2027 limits rise with the federal poverty level. MassHealth makes the final decision.
Watch the overlap: The April 2026 rate guide says people enrolled in the Medicare Savings Program cannot also be in Prescription Advantage. For most, Extra Help through the MSP is the richer benefit. Details on the federal side are on the Extra Help 2027 page.
$7,500
total yearly drug cost at which the standard 2027 design reaches the cap (our math)
$2,400
most you pay for covered drugs in 2027, then $0
$200
monthly bill if the full $2,400 is spread over 12 months
The $7,500 comes from the CMS parameters: the $700 deductible, plus 25% of the next $6,800 in drug costs ($1,700), equals $2,400. Plans that use flat tier copays reach the cap at a different point. Former Prescription Advantage applicants who were counting on a state cap can use the Medicare Prescription Payment Plan to turn a large January bill into monthly installments. Premiums never count toward the cap.

Missing from our Massachusetts count
SilverScript: KFF reports SilverScript Choice is ending in 13 Part D regions for 2027, and no SilverScript plan appears in our Massachusetts count
HealthSpring Extra Rx: only Assurance Rx is filed for this region in our count
Medicare Rx Preferred from UHC: UnitedHealthcare filed only the Saver plan here
Aetna: no Aetna-branded standalone drug plan is filed for the region in our count, though Aetna still sells Medicare Advantage in parts of the state
Anyone whose drug plan was not renewed should have received a letter. That letter opens a special enrollment period, explained on the Part D special enrollment period 2027 page. Choosing a new plan by December 31, 2026 avoids a gap on January 1.
Write down every drug and dose. Include inhalers, insulin and anything bought at a mail-order pharmacy.
Find a SHINE counselor. SHINE, short for Serving the Health Insurance Needs of Everyone, is a free state program with trained counselors at local councils on aging.
Run the list through Plan Finder. Compare total yearly cost, not just the premium, across the 9 plans and your preferred pharmacy.
Ask about MSP and Extra Help. If income is near the MSP limit, apply through MassHealth; there is no asset test.
Enroll by December 7, 2026. Coverage starts January 1, 2027.
The Bottom Line
Massachusetts has 9 standalone drug plans for 2027 in our count, the same list found across Connecticut, Rhode Island and Vermont. With Prescription Advantage closed to new applicants, newcomers should lean on careful plan comparison and on the no-asset-test Medicare Savings Program for extra help. SHINE can walk through the choices at no charge.

Not as a new member. Mass.gov says the program stopped accepting new applications after September 11, 2026, though people already enrolled continue.
Nine, in our count of CMS 2027 plan filings, from Humana, Blue MedicareRx, Wellcare, HealthSpring and UnitedHealthcare. Plan Finder may show a slightly different number.
Blue Cross Blue Shield of Massachusetts markets the two Blue MedicareRx plans, Premier and Value Plus. They are the only Blue drug-only plans in our Massachusetts count.
No. Standalone plans cover the whole state, so the same 9 are offered from Boston to the Berkshires, though pharmacy pricing can vary.
No. MassHealth eliminated the MSP asset limit in March 2024, so only income is counted, and MSP members get Extra Help automatically.
Not in our count. KFF reports SilverScript Choice is ending in 13 regions for 2027, so former members need a new plan.
Massachusetts uses its own Medigap plan names, and policies sold today do not include drug coverage. A separate Part D plan is usually needed to avoid a late penalty.
SHINE offers free counseling through local councils on aging and senior centers, and counselors do not sell insurance.
